Download and fill out Enrollment and Emergency Forms 2. Copy Immunization Records. 3. Download and fill out Tuition Calendar. 4. Bring Registration fee ($35/child; $70/family) along with tuition payment on the first day of care. You must also bring Enrollment/Emergency Forms and immunization records by the first day of care. 5. If you believe you may qualify for CCAP, call 303-604-1043 for Jefferson County and 720-944-KIDS for Denver County to see about approval.
